In-depth history review, goals discussed, treatment plan + testing/script sent after consult.
Service Description
Rachael will gather in depth information about your/your child's medical history, current health issues and priorities. She will also take record of, if any, current pharmaceutical medication and past pathology so please have past records available or send before the appointment. She will also discuss your particular health goals/goals for your child, dietary and lifestyle habits. Further pathology may be requested and ordered during this appointment After the consult you will be sent an individualised treatment plan and if required a prescription through Vital.ly.
Cancellation Policy
Please allow 48 hours notice to reschedule your appointment. No shows incur a full fee of the appointment.
